Home-Based Services

Our Home-Based Services program provides comprehensive, individualized support to individuals with intellectual and developmental disabilities in the comfort of their own homes. Our Self-Directed Assistants work collaboratively with individuals with intellectual and developmental disabilities and their family members, caregivers, therapists, and support staff to develop customized plans that address their unique needs and goals.

Our home-based services program includes a range of services, such as personal care assistance, therapy services (including physical, occupational, and speech therapy), behavioral support, mobility modifications, home modifications, and/or life skills training. We believe in empowering individuals to live their best lives by providing the support they need to thrive at home and in their communities. We are dedicated to promoting independence, inclusion, and quality of life for all of our clients.

Enrollment

To enroll in our home-based services program at UCP-Center for Disability Services, contact Cassandra Farley at cassandraf@ucp-cds.org or by calling (815) 744-3500. Cassandra will be able to provide you with more information about our services and answer any questions you may have. 

Please know that in order to receive home-based services from UCP-Center for Disability Services, you must first go through your local Independent Service Coordination agency. For residents of Will and Kankakee counties, this agency is Service, Inc. of Illinois. If you don't live in Will or Kankakee counties, please contact us and we will refer you to your Service Coordination agency or click here to search for your local agency.

Staff members at Service, Inc. of Illinois will inform you about the requirements for formally requesting home-based services from UCP-Center for Disability Services and help you initiate and facilitate the enrollment process. 

Once you have been referred to us by your Independent Service Coordination agency, our team will work with you to determine the specific services you need and develop a personalized plan to support your needs and goals. 

Throughout your time with us, our dedicated staff will provide ongoing support and assistance to ensure that you are able to live comfortably and independently in your own home.
